PAYMENT OPTION PLAN
The payment option plan is set up for the students that wish to make payments.The
first payment of $215.00 for 2 payments, or $150.00 for 3 payments is due with
registration. The second payment of $215.00 for 2 payments or $150.00 for 3
payments is due on ________(put in date 2 weeks later). The final payment of $0 for
2 payments or $130.00 for 3 payments is due on_______(put in date 2 weeks after
2nd payment. All payments must be paid by the second to the last of class, otherwise
the student is automatically dropped from the class. Any student wishing to drop the
class proir to the start of the first day of class shall receive their deposit minus 10%.
This will be sent to you in the mail. The student enters into this contract willingly
and agrees to make payments as stated.
