CARING HEARTS HEALTH CARE ADMISSION AGREEMENT
1. Students are expected to attend all class and clinical days. Failure to do so
may effect your grade and progress in the class. Make up time or homework maybe required.
2. We maintain a respectful atmosphere in class and at our clinical site. All
students must follow this policy towards other students, instructors, residents
and their families and the staff at our clinical site. Any student showing disregard
for this policy will be dismissed from the class.
3. Student agrees to obtain a BCI background check prior to going to clinical. FBI
background checks are required if you have lived in Ohio less than 5 years.
4. Student agrees to obtain TB testing as required prior to going to clinical.
5. All fees must be paid in full prior to attending class. We offer a payment plan
(POP) and fees must be paid as stated in contract. Failure to do so will drop student
from class.
6. Student states that she/he is physically able to preform tasks such as lifting etc.
as required for nurse aide training.
7. Student states and agrees that she/he is fluent in written and spoken English.
8. The price of $430.00 includes your book, workbook, skills pamphlet, class, clinical, and skills review.
